Many accidents, often leading to injury, occur throughout the UK every day and power generation organisations are no exception.
The failure of people, equipment, supplies, or surroundings to behave or react as expected causes most of them. Investigating these can determine how and why these failures occur. By learning from the information gained through an investigation, a similar adverse event –perhaps with worse consequences – may be prevented. It is important to conduct such investigations with prevention in mind.
